Family Transition Partner Job Description

Position Title: Family Transition Partner (FTP)

Reports to: Local FTP Supervisor and the State FTP Coordinator

Job Overview

The Prevention Pilot Project FTP will provide support and coordination to families and children in the Families in Transition Teams (FiTT). The purpose of Families in Transition Teams is to assist families and children experiencing major life transition such as returning home from inpatient care, being placed in foster care or adoption. The FTP will walk alongside the family and child as they move through the transition they are facing. They will support them to access needed community services, to implement prevention/crisis management strategies, and will monitor their progress and ongoing needs. They will support them in making changes in their family support plan as their needs change over time.

FTPs must be committed to providing high quality supports that focus on the families’ needs and priorities in a manner that is honest and respectful.

Candidates should have the following qualifications and experience:

Education:

• Minimum of a bachelor’s degree in social services, education, psychology or related field

• Years of experience can be substituted for educational requirement

Experience:

• experience working with children/families experiencing difficulty

Additional Requirements:

• Must have a heart for supporting children and families in their community to enjoy a better quality of life

• Must have strong people skills and be able to form effective working relationships with a wide range of people

• Must have the ability to listen and problem solve

• Must have communication skills that enable effective, courteous interaction with families and team members

• Must pass required background checks and drug screen

• Must be able to lift to 30 pounds and climb stairs

• Must have a valid driver's license and reliable transportation, travel is required

• Must be available evenings, weekends and on-call as assigned

• Must be willing to complete required training
